Prescription



Main formula



Jing fang bai du san


Schizonepeta and Ledebouriella Tablet to Overcome Pathogenic Toxin. This formula is indicated in invasion of Wind-Cold to the Exterior portions of the body. Although it has good properties which relieve the External symptoms, its function to relieve the painful swallowing is not sufficient; some other herbs should be prescribed to relieve the painful swallowing, such as Gui Zhi Ramulus Cinnamomi 6g, She Gan Rhizoma Belamcandae Chinensis 10g and Xuan Fu Hua Flos Inulae 10g to benefit the throat, descend the Stomach-Qi and relieve the painful swallowing.


Alternative formulas



Ma huang tang


Ephedra Decoction. This formula dispels Wind-Cold, indicated in painful swallowing due to severe invasion of Wind-Cold with lack of sweating. Chuan Xiong Radix Ligustici Wallichii 10g, Xi Xin Herba Asari 5gand Jie Gen Radix Platycodi 5g could be added to relieve the pain.


Gui zhi tang


Cinnamon Twig Decoction. Compared with the above formulas, this decoction is not so effective at dispelling Wind-Cold, so it is indicated in painful swallowing due to slight severe invasion of Wind-Cold with lack of slight sweating. The same modifications as above could be used.


Modifications





1. If there is swelling of the throat, add Bai Bu Radix Stemmonae 10g and Jiang Can Bombys Batryticatus 10g to regulate the Qi and reduce the swelling


2. If there is a severe aversion to cold, add Ma Huang Herba Ephedrae 10g and Xi Xin Herba Asari 3g to dispel Wind-Cold and relieve the External symptoms


3. If there is a much fever, add Chai Hu Radix Bupleuri 5g to dispel Cold and reduce the fever


4. If there is much headache, add Bai Zhi Radix Angelicae Dahuricae 10g and Man Jing Zi Fructus Viticis 10g to dispel Wind and Cold and sedate the headache


5. If there is a severe cough and expectoration of white phlegm due to invasion of the Lung by Wind-Cold, add Zi Wan Radix Asteris 10g and Xing Ren Semen Armeniacae Amarum 10g to dispel the Wind-Cold, disperse the Lung-Qi and relieve the cough.

Main Points


TE-5 Waiguan, LI-4 Hegu, BL-12 Fengmen, BL-17 Geshu, CV-17 Tanzhong and CV-18 Yutang. Reducing method is used on these points. Moxibustion should be used on the first three points.



Explanations





▪ LI-4 and TE-5, the Source point and the Connecting point of the Large Intestine channel and Triple Burner channel respectively, promote the sweating and relieve the External symptoms due to the invasion of Wind-Cold.


▪ BL-12 opens the skin pores and promotes sweating so as to relieve blockage of the Lung system resulting from invasion of External factors.


▪ BL-17, the Back Transporting point of the diaphragm, relaxes the diaphragm, promotes descent of the Stomach-Qi and relieves painful swallowing. It also promotes the circulation of Blood and eliminates Blood stasis.


▪ CV-17, the Gathering point for the Qi, promotes the circulation of Qi and relieves its stagnation in the chest and relieve blockage in the throat, oesophagus and Stomach.


▪ CV-18 promotes swallowing and relieves the painful swallowing.

BL-13 Feishu, LU-1 Zhongfu, LU-5 Chize, LU-6 Kongzui, ST-9 Renying, ST-10 Shuitu, BL10 Tianzhu and CV-17 Tanzhong, etc.


Modifications





1. If there is swelling in the throat, and much pain on swallowing, add CV-22 and CV-23 in order to reduce the swelling and relieve the pain


2. If there is nausea or vomiting, add PC-6, the Confluent point of the Yin Linking Vessel, to harmonise the Stomach, cause the Stomach-Qi to descend and relieve the vomiting


3. If there is a cough with much white phlegm, add LU-7, the Connecting point of the Lung channel, to disperse the Lung-Qi and relieve the cough


4. If there is generalised body pain with a cold feeling, add BL-58 and BL-63, the Connecting point and the Accumulation point of the Bladder channel respectively, to dispel the Wind-Cold and relieve the body pain


5. If there is hoarseness, add LU-11, the Well point of the Lung channel, to open the collateral of the Lung and improve the voice


6. If there is headache, add Extra Yintang and GV-16 to dispel the Wind-Cold and stop the headache.





Invasion of Wind-Heat



Symptoms and signs


Symptoms and signs of invasion of Wind-Heat are acute onset of painful swallowing, a burning feeling, swelling and redness in the throat, with formation of pus, vomiting, accompanied by a slight fever, an aversion to cold and wind, a runny nose with a yellow discharge, headache, sweating, thirst, restlessness, generalised body pain, a thin and yellow tongue coating and a superficial and rapid pulse.


Principle of treatment


Dispel the Wind, clear the Heat, reduce the swelling and relieve the pain.





Prescription



Main formula



Yin qiao san


Honeysuckle and Forsythia Powder. This formula is indicated in invasion of Wind-Heat or Toxic Heat. When it is used to treat the painful swallowing, some other herbs should be added into the prescription, such as Ye Ju Hua Flos Chrysanthemi Indici 10g, Chuan Xiong Radix Ligustici Wallichii 10g and Huang Qin Radix Scutellariae 10g, etc, to relieve the pain.

Sang ju yin


Mulberry Leaf and Chrysanthemum Decoction. This formula is mainly indicated in tongue pain due to invasion of Wind-Heat to the Lung system, manifested mainly as cough and throat pain with painful swallowing. Some other herbs, such as Ye Ju Hua Flos Chrysanthemi Indici 10g, Chuan Xiong Radix Ligustici Wallichii 10g, Huang Qin Radix Scutellariae 10g, etc., should be added to strongly relieve the pain.


Pu ji xiao du yin


Universal Benefit Decoction to Eliminate Toxin. This formula clears the Heat, removes the Toxin and reduces the swelling, indicated in painful swallowing due to invasion of Toxic Heat. Some other herbs could be added such as Jin Yin Hua Flos Lonicerae Japonicae 10g, Pang Da Hai Semen Sterculiae Scaphigerae 5 pieces, Pu Gong Ying Herba Taraxaci Mongolici cum Radice 10g and Ye Ju Hua Flos Chrysanthemi Indici 10g to increase the therapeutic results to control the pain.


Modifications





1. If there is a high fever, add Shi Gao Gypsum Fibrosum 20g and Da Qing Ye Folium isatidis 15g to clear the Fire and reduce the fever


2. If there is severe swelling and pain in the throat, add Sheng Di Huang Radix Rehmanniae 10g to remove the Toxin and reduce the swelling


3. If there is much headache, add Gou Teng Ramulus Uncariae cum Uncis 10g and Shi Jue Ming Concha Haliotidis 15g to eliminate the Wind-Heat and relieve the headache


4. If there is severe constipation, add Mang Xiao Natrii Sulfas 10g and Da Huang Radix et Rhizoma Rhei 6g to clear the Excess-Fire and eliminate the Toxin by means of defecation.


5. If there is much thirst, add Sha Shen Radix Adenophorae 10g to promote the secretion of Body Fluid and relieve the thirst.


Patent remedy


Yin Qiao Jie Du Pian Honeysuckle and Forsythia Tablet to Overcome Toxins.






Main Points


LI-4 Hegu, LI-11 Quchi, LU-10 Yuji, BL-12 Fengman, GV-14 Dazhui, CV-18 Yutang and CV-22 Tiantu. Reducing method is used on these points.



Explanations





▪ LI-4 and LI-11, the Source point and the Sea point of the Large Intestine channel respectively, and GV-14 dispel the Wind, clear the Heat and reduce the fever so as to relieve External symptoms due to the invasion of Wind-Heat.


▪ BI-12 helps the above-mentioned points to clear the Wind-Heat and relieve the External symptoms.


▪ LU-1o clears the Heat, subsides the swelling and relieves the painful swallowing.


▪ CV-18 and CV-22 promote the swallowing and reduce the swelling in the throat so as to relieve painful swallowing.

BL-13 Feishu, LU-1 Zhongfu, LU-5 Chize, LU-6 Kongzui, LU-11 Shaoshang, ST-9 Renying, ST-10 Shuitu, LI-2 Erjian and CV-17 Tanzhong, etc.


Modifications





1. If there is a high fever and severe painful swallowing due to swelling in the throat, use bleeding method at LI-1 and LU-11 to clear the Heat, reduce the fever and relieve the throat pain


2. If there is pain behind the chest on swallowing, add CV-21 to reduce the swelling strongly and promote swallowing


3. If there is nausea or vomiting, add PC6, the Confluence point of the Yin Linking Vessel, and CV-12, the Gathering point for the Fu organs and the Front Collecting point of the Stomach, to harmonise the Stomach and cause the Stomach-Qi to descend so as to relieve the vomiting


4. If there is a cough with yellow phlegm, add BL-13, the Back Transporting point of the Lung, and LU-1, the Front Collecting point of the Lung, to disperse the Lung-Qi and relieve the cough


5. If there is chest pain due to cough, add LU-6, the Accumulation point of the Lung channel, to disperse the Lung-Qi and sedate the pain


6. If there is much thirst and a dry mouth, add ST-44 and LI-2, the Spring points of the Stomach channel and Large Intestine channel respectively, to clear the Heat and preserve the Body Fluids


7. If there is severe constipation, add ST-25, the Front Collecting point of the Large Intestine, and ST-37, the Lower Sea point of the Large Intestine, to promote the defecation and relieve the constipation.





A 14-year-old boy had been suffering from painful swallowing for 1 week. He had fallen ill with acute pharyngitis 10 days previously and had been given antibiotics. The swelling in his throat had improved, but his throat was painful when he swallowed food. When he arrived at the clinic, he had a slightly swollen throat. He also had a thirst, a bitter taste in his mouth, constipation, a red tongue with a dry yellow coating and a pulse.


Diagnosis


Invasion of External Wind-Heat.

▪ Lian Qiao, Niu Bang Zi and Jiang Can dispel Wind, clear Lung-Heat and relieve the pain


▪ Zhi Qiao regulates the Qi circulation and relieves pain.

The boy had a daily treatment with this herbal decoction. His painful swallowing was relieved after 1 week of treatment. His father was followed up by letter a year later. He stated that his son had experienced no painful swallowing since the herbal treatment.


Invasion of Wind-Dryness



Symptoms and signs


Symptoms and signs of Wind-Dryness are acute onset of painful swallowing, dryness in the mouth and throat and on the lips, with no obvious swelling in the throat, accompanied by a slight aversion to cold, a low-grade fever, thirst, dry skin, a dry cough, scanty phlegm expectoration, a slight headache, a thin and dry tongue coating and a superficial pulse.

If the cause is Wind-Heat-Dryness, there will be expectoration of yellow phlegm, possibly with blood in the phlegm, a red tongue with a thin, yellow and dry coating and a superficial and rapid pulse.

If the cause is Wind-Cold-Dryness, there will be expectoration of white phlegm, itching in the throat, a runny nose with a white discharge, a thin, white and dry tongue coating and a superficial and slow pulse.


Principle of treatment


Dispel the Wind, moisten the Dryness, reduce the swelling and relieve the pain.






A. Prescription for wind-cold-dryness



Xing su san


Apricot Kernel and Perilla Leaf Powder. This formula has functions to dispel Wind-Cold, disperse the Lung-Qi, relieve the Dryness in the throat and stop the cough. It also has some effect to relieve the pain swallowing due to invasion of Wind-Cold-Dryness. Pang Da Hai Semen Sterculiae Scaphigerae 5 pieces, Mai Men Dong Radix Ophiopogonis 10g, SHE GAN Rhizoma Belamcandae Chinensis 10g and Xuan Fu Hua Flos Inulae 10g could be added into the prescription to relieve the pan.


B. Prescription for wind-heat-dryness



Sang xing tang


Mulberry Leaf and Apricot Kernel Decoction. This formula dispels the Wind, Heat and Dryness, nourishes the Body Fluid, benefits the throat, and relieves the cough and painful swallowing. Huang Qin Radix Scutellariae 10g, Pang Da Hai Semen Sterculiae Scaphigerae 5 pieces, Mai Men Dong Radix Ophiopogonis 10g and Xuan Fu Hua Flos Inulae 10g could be added to relieve the painful swallowing.


Modifications





1. If there is a severe swollen throat due to the invasion of Wind-Heat, add Pu Gong Ying Herba Taraxaci 10g and Zhu Ye Herba Lophatheri 10g to clear the Heat and reduce the swelling


2. If there is a thirst due to the invasion of Dry-Heat, add Xuan Shen Radix Scrophulariae 10g to nourish the Yin of the Lung and the Stomach and relieve the thirst


3. If there is a fever due to the predominance of Dry-Heat invasion, add Chai Hu Radix Bupleuri 10g and Sheng Ma Rhizoma Cimicifugae 10g to clear the Heat and reduce the fever


4. If there is much headache due to the disturbance of the clear Yang in the head by Wind-Heat, add Chuan Xiong Rhizoma Ligustici Chuanxiong 10g and Man Jing Zi Fructus Viticis 10g to dispel the Wind-Heat and sedate the headache


5. If there is a dry cough with itching in the throat, add Tian Hua Fen Radix Trichosanthis 10g and Chuan Bei Mu Bulbus Fritillariae Cirrhosae 10g to disperse the Lung-Qi, nourish the Yin and dispel the Dryness.


Patent remedy


Qing Fei Yi Huo Pian Clear Lungs Restrain Fire Tablet.






Main points


LI-4 Hegu, BL-12 Fengmen, BL-13 Feishu, LU-7 Lieque, LU-8 Jingqu and ST-45 Lidui. Reducing method is used on these points, except for LU-8 and LU-5, on which a tonifying method is used.


Explanations





▪ LI-4, the Source point of the Large Intestine channel, and BL-12 dispel the External pathogenic factors in order to relieve the External symptoms.


▪ LU-7 and BL-13, the Connecting point of the Lung channel and the Back Transporting point of the Lung respectively, dispel the External factors from the Lung system, and harmonise the collateral of the Lung in order to clear away pathogenic factors in the throat.


▪ LU-8 and ST-45, the Metal points of Lung channel and Stomach channel respectively, are used here to promote the secretion of Body Fluids in the Lung and Stomach so as to moisten the throat, oesophagus and stomach and relieve the Dryness.
