Main formula



Tian tai wu yao san


Top-Quality Lindera Powder. This formula can dispel the Cold, warm the channel, harmonise the collaterals and relieve the genital pain or abdominal pain, and is especially used to treat the genital pain caused by invasion of Wind-Cold to the Liver channel. Chuan Xiong Radix Ligustici Chuanxiong 10g and Huai Niu Xi Radix Achyranthis Bidentatae 10g could be added to the prescription.

If there is genital pain due to invasion of Cold-Damp, Qin Jiao Radix Gentianae Macrophyllae 10g and and Du Huo Radix Angelicae Pubescentis 10g should also be combined with to eliminate Cold-Damp.

Dang gui si ni tang


Angelica Four Rebellious Decoction. This formula can be used to warm the channel, dispel the Cold, promote the Qi circulation and relieve the pain. Wu Yao Radix Linderae 5g, Ju He Semen Citri Reticulatae 10g, Bing Lang Semen Arecae 5g, Chuan Xiong Radix Ligustici Chuanxiong 10g and Huai Niu Xi Radix Achyranthis Bidentatae 10g could be applied in the prescription to relieve the pain.


Du huo ji sheng tang


This formula is indicated in genital pain due to deficiency of Liver and Kidney with invasion of Wind, Cold and Damp. Chuan Xiong Radix Ligustici Chuanxiong 10g, Li Zhe He Semen Litchi Chinensis 10g, Gui Zhi Ramulus Cinnamomi Cassiae 10g, Xiang Fu Rhizoma Cyperi 10g and Yu Jin Radix Curcumae 10g should be added to relieve the pain.


Juan bi tang


Remove Painful Obstruction Decoction. This formula has functions which dispel Wind, Cold and Damp, promote the Qi and Blood circulating in the channels and collaterals and relieve the pain. It is especially indicated in either acute or chronic lower back pain and joint pain due to invasion of Wind, Cold and Damp with deficiency of Qi and Blood. Chuan Xiong Radix Ligustici Chuanxiong 10g, Li Zhe He Semen Litchi Chinensis 10g, Gui Zhi Ramulus Cinnamomi Cassiae 10g, Xiang Fu Rhizoma Cyperi 10g and Qing Pi Pericarpium Citri Reticulatae Viride 5g should be added to relieve the pain.


Modifications





1. If there is severe cold, add Rou Gui Cortex Cinnamomi 6g and Wu Zhu Yu Fructus Evodiae 5g to warm the Liver channel and to relieve the pain


2. If there is dysmenorrhoea, add Hong Hua Flos Carthami 10g and Dang Gui Radix Angelicae Sinensis 10g to remove the Blood stasis and to regulate the menstruation


3. If there is severe pain in the testicles, add Ju He Semen Citri Peticulatae 10g and Li Zhi He Semen Litchi 10g to regulate the circulation of Qi in the Liver channel and to relieve the pain


4. If there is lower abdominal pain, add Gan Jiang Rhizoma Zingiberis 6g and Yan Hu Suo Rhizoma Corydalis 10g to warm the Lower Burner in order to relieve the pain


5. If there is a swollen scrotum, add Fu Ling Poria 15g, Ze Xie Rhizoma Alismatis 10g and Che Qian Zi Semen Plantaginis 10g to promote the urination and to eliminate the Damp.

Main points


LI-4 Hegu, TE-5 Waiguan, LR-3 Taichong, LR-5 Ligou and SP-6 Sanyinjiao. Reducing method is used on these points. Moxibustion is recommended on LR-3 and LR-5.


Explanation





▪ LI-4 and TE-5 dispel External the Cold and relieve the External symptoms.


▪ LR-3, the Stream and Source point of the Liver channel, warms the channel and regulates the circulation of Qi. LR-5, the Connecting point of the Liver channel, eliminates the External Cold, harmonises the collateral and relieves pain. These two points are both treated with moxibustion to warm the Liver channel and to dispel the External Cold.


▪ SP-6, the crossing point of the three Yin channels of the leg, regulates the circulation of Qi and Blood and relieves the pain.


Alternative points


LR-1 Dadun, LR-4 Zhongefng, LR-6 Zhongdu, LR-8 Ququan, LR-12 Jimai, GV-2 Yaoshu, GV-4 Yaoyangguan and CV-2 Qugu, etc.


Modifications





1. If there are painful testicles, add LR-8 and LR-12 to regulate the circulation of Qi in the Liver channel and to relieve the pain.


2. If there is a painful vagina, add ST-29 and CV-2 to warm the channel and to relieve the pain.


3. If there are swollen genitals, add ST-30 and SP-9 to eliminate the Damp and to relieve the swelling.


4. If there is lower abdomen pain, add CV-3 and ST-28 to warm the channels and to relieve the pain. At the same time, moxibustion treatment on CV-4 is recommended.


5. If there is perineal pain, add CV-1 and KI-10 to regulate the circulation of Qi and to relieve the pain.





A 43-year-old man came to the acupuncture department with pain at the right side of his lower abdomen, which he had had for the past 20 days. The pain had an acute onset: it was there one morning when he woke up in an air-conditioned room. The pain radiated from the genitals to the interior aspect of the right thigh and there was a slight feeling of stiffness and pain with a cold sensation in the scrotum. He had already visited a doctor who gave him some painkillers and advised bed rest. He was known to have an inguinal hernia on his right side, but it had never been operated on. His tongue was white and moist, and his pulse was superficial and wiry.


Diagnosis


Invasion of External Cold to the Liver channel.


Principle of treatment


Warm the Liver channel, eliminate the External Cold and relieve the genital pain.


Acupuncture treatment


LR-3, LR-8, LR-12, ST-29 and CV-4 were needled with reducing method. Treatment was given once every other day.


Explanations





▪ LR-3, the Source point of the Liver channel, and LR-8, the Sea point, regulate the Liver-Qi and dispel External Cold in the channel


▪ LR-12, the local point, harmonises the collateral and stops the pain


▪ CV-4 warms the channels and regulates the circulation of Qi in the local area, especially for genital disorders


▪ ST-29 circulates the Qi in the genital region and relieves the pain.

After 2 weeks, the genital pain was completely relieved. Upon follow-up a year later, the man reported that he had experienced no further genital pain since the treatment.





Invasion of excessive-fire to the terminal yin channel



Symptoms and signs


A sharp pain with a hot sensation in the genitals, redness, swelling and a burning feeling in the pudendum, fever, thirst, constipation, yellow leucorrhoea, dark yellow urine, restlessness, a red tongue with a yellow and dry coating and a rapid and slippery pulse are the symptoms of this condition.

Clear the Heat, reduce the Fire, remove the Toxin and stop the pain.

Main formula



Wu wei xiao du yin


Five-Ingredient Decoction to Eliminate Toxin. This formula is very good at eliminating the Toxin and reducing the swelling. Chuan Xiong Radix Ligustici Wallichii 10g, Sheng Di Huang Radix Rehmanniae 10g and Tu Fu Ling Rhizoma Smilacis Glabrae 10g could be added to relieve the pain.

Huang lian jie du tang


This formula is good for clearing the Toxic and reducing the Fire, but its effect on the pain is not sufficient, so some other herbs have to added, such as Chuan Xiong Radix Ligustici Wallichii 10g, Yu Jin Tuber Curcumae 10g, and Bai Zhi Radix Angelicae Dahuricae 10g, etc., to relieve the pain.


Modifications





1. If there is a high fever, add Shi Gao Gypsum Fibrosum 30g, Da Qing Ye Folium isatidis 10g and Zhi Mu Rhizoma Anemarrhenae 10g to clear the Fire and to reduce the fever


2. If there is severe swelling and pain, add Xuan Shen Radix Scrophulariae 10g to remove the Toxin, reduce the swelling and to relieve the pain


3. If there is itching of the genitals, use Bai Xian Pi Cortex Dictamni Radicis 10g, Ku Shen Radix Sophorae Flavescentis 10g and She Chuang Zi Fructus Cnidii 10g as a lotion to wash the genitals twice a day in order to relieve the itching, reduce the swelling and stop the pain


4. If there is constipation, add Mang Xiao Natrii Sulfas 10g and Da Huang Radix et Rhizoma Rhei 6g to clear the Excessive-Fire and to eliminate the Toxin by means of defecation.

Niu Huang Jie Du Pian Cattle Gallstone Tablet to Resolve Toxin.

▪ LR-3, the Stream and Source point of the Liver channel, promotes the circulation of Liver-Qi and reduces the Liver-Fire


▪ LR-8, the Sea point and the Water point of the Liver channel, clears the Fire in the channel


▪ SP-6, the crossing point of the Spleen, Kidney and Liver channels, and SP-9, the Sea point of the Spleen channel, reduce the Fire and regulate the circulation of Qi in the Lower Burner


▪ KI-10, the Sea point of the Kidney channel, nourishes the Kidney-Yin in order to clear the Liver-Fire


▪ CV-2, the point where the Liver channel and the Directing Vessel cross, reduces Liver-Fire and relieves the redness and swelling in the genitals


▪ PC-6, the Connecting point of the Pericardium channel and Confluence point of the Yin Linking Vessel, tranquillises the Mind and clears the Liver-Fire.

LR-1 Dadun, LR-2 Xingjian, LR-5 Ligou, LR-12 Jimai, GV-2 Yaoshu, LI4 Hegu, LI-11 Quchi, etc.


Modifications





1. If there is fever due to the flaring up of Liver-Fire, add LR-2, the Spring point of the Liver channel, LI-4 and LI-11, the Source point and the Sea point of the Large Intestine channel respectively, to reduce the Liver-Fire and clear the fever


2. If there is redness and swelling of the genitals, add ST-30 to reduce the Fire in order to relieve the redness and swelling


3. If there is painful urination, add KI-2, the Spring point of the Kidney channel, to clear the Heat, promote the urination and relieve the pain


4. If there is restlessness, add HT-3 and HT-8, the Source point and the Spring point of the Heart channel respectively, to reduce the Fire and tranquillise the Mind


5. If there is constipation, add ST-25, the Front Collecting point of the large Intestine, and ST-37, the Lower Sea point of the Large Intestine, to promote the circulation of Qi and to relieve the constipation.
