Main formula



Juan bi tang


Remove Painful Obstruction Decoction. This formula can dispel the Wind, eliminate the Cold, resolve the Damp, promote the Qi and Blood circulation and relieve the pain indicated in toe pain due to invasion of Wind, Cold and Damp with stagnation of Qi and Blood. Chuan Xiong Radix Ligustici Chuanxiong 10g, Hai Feng Teng Caulis Piperis Futokadsurae 10g, Du Huo Radix Angelicae Pubescentis 10g, Huai Niu Xi Radix Achyranthis Bidentatae 10g, and Sang Zhi Fructus Gardeniae 10g should be added to relieve the pain.


Alternative formulas



Yi yi ren tang


Coicis Decoction. This formula has a strong function which dispels the Wind, eliminates the Cold, resolves the Damp and relieves the pain, and is especially indicated in toe pain due to invasion of Wind, Cold and Damp.


Ma xing yi gan tang


Ephedra Apricot Kernel, Coicis and Licorice Decoction. This formula can treat toe pain in the early stage due to invasion of Wind, Cold and Damp. It can only eliminate Wind, Cold and Damp in general; it is incapable of relieving the toe pain specifically.

Huai Niu Xi Radix Achyranthis Bidentatae 10g, Qin Jiao Radix Gentianae Macrophyllae 10g, Rou Gui Cortex Cinnamomi 6g, Cang Zhu Rhizoma Atractylodis 12g, Qiang Huo Rhizoma et Radix Notopterygii 12g and Du Huo Radix Angelicae Pubescentis 10g could be added to the prescription to eliminate the External pathogenic factors and relieve the toe pain.


San ren tang


Three-Nut Decoction. This formula can treat invasion of Wind, Damp and Heat, in which Damp is the predominant pathogenic factor. Luo Shi Teng Caulis Trachelospermi 15g, Sang Zhi Ramulus Mori Albae 15g, Xi Xian Cao Herba Siegesbeckiae 10g, Chuan Xiong Radix Ligustici Chuanxiong 10g and Hai Feng Teng Caulis Piperis Futokadsurae 10g should be added to dispel the Wind, to eliminate the Damp-Heat and relieve the pain.


Jia wei xuan bi tang


Disband Painful Obstruction Decoction. This formula dispels the Wind, clears the Heat, eliminates the Damp and relieves the pain indicated in toe pain due to invasion of Wind, Damp and Heat. Jiang Huang Rhizoma Curcumae Longae 15g, Hai Tong Pi Cortex Erythrinae 15g and Sang Zhi Ramulus Mori 15g, etc., could be added into the prescription to relieve the pain.


Modifications





1. If there is spasm of the toe due to predominance of Cold invasion, add Bai Zhi Radix Angelicae Dahuricae 10g to eliminate the Cold and relieve the spasm


2. If there is swelling of the toes, add Du Huo Radix Angelica Pubescentis 10g and Huai Niu Xi Radix Achyranthis Bidentatae 10g to dispel the Damp, reduce the swelling and sedate the pain


3. If there is swelling and redness of the toes, add Zhi Mu Rhizoma Anemarrhenae 10g and Fang Ji Radix Stephaniae Tetrandrae 10g to clear the Heat and remove the obstructions from the collateral


4. If there is severe fever and chills, add Gui Zhi Ramulus Cinnamomi 10g to relieve the External syndrome and reduce the fever


5. If there is a headache due to Wind-Cold invasion, add Chuan Xiong Rhizoma Ligustici Chuanxiong 6g to dispel the Wind-Cold and relieve the headache


6. If there is a poor appetite due to invasion of Wind-Cold, add Ban Xia Rhizoma Pinelliae 10g and Chen Pi Pericarpium Citri Reticulatae 10g to improve the appetite.

Han Shi Bi Chong Ji Cold-Damp Bi Syndrome Infusion.







Main points


TE-5 Waiguan, LI-4 Hegu, BL-12 Fengmen, BL-60 Kunlun, GB-40 Qiuxu, SP-6 Sanyinjiao and Extra Bafeng, as well as some local Ah Shi points. Reducing method is used on these points.


Explanations





▪ TE-5, LI-4 and BL-12 dispel External pathogenic Wind and relieve the External symptoms


▪ BL-60 and GB-40 are suitable for regulating the local circulation of Qi and Blood


▪ SP-6 is the crossing point of Kidney, Spleen, Liver channels, and is good at regulating the Blood and Qi


▪ Extra Bafeng as well as the Ah Shi points dispel Wind, Cold and Damp and promote the Qi and Blood circulation on the feet so as to relieve the toe pain.

LU-7 Lieque, TE-5 Waiguan, GB-34 Yanglingquan, GB-43 Xiaxi and Extra Baxie, etc.


Modifications





1. If there is fever and chills, add GV-14, the meeting point for all the Yang channels, to dispel the Wind, relieve External symptoms and reduce the fever


2. If there is swelling of the foot and toes, add SP-9, the Sea point of the Spleen channel, to remove the Damp and reduce the swelling


3. If there is redness, swelling and a hot sensation in the toe, add LI-11, the Sea point of the Large Intestine channel, and ST-44, the Spring point of the Stomach channel, to clear the Heat and reduce the swelling


4. Some corresponding points on the finger should be added in order to increase the therapeutic effect


5. If there is a headache, add GB-20 to dispel the Wind and relieve the headache


6. If the appetite is poor, add SP-4 and ST-36 to activate the Spleen and improve the appetite.





A 50-year-old woman had been suffering from toe pain for 3 years. She was diagnosed as having ‘rheumatoid arthritis’, and often took painkillers and anti-inflammatory drugs to relieve the toe pain.

During the previous 2 weeks, she had been suffering a lot of toe pain after an infection, and was unable to sleep because of the pain. Besides the toe pain, she also had generalised body pain, a slight fever (37.5°C) during the day, an aversion to cold, a headache, a thin and white tongue coating, and a superficial and wiry pulse.


Diagnosis


Toe pain due to invasion of Wind-Cold-Damp.


Principle of treatment


Dispel Wind, expel Cold, eliminate Damp, smooth the channels and collateral and relieve the pain.

Fang Feng Radix Ledebouriellae 10g


Ge Gen Radix Puerariae 15g


Ma Huang Herba Ephedrae 6g


Dang Gui Radix Angelicae Sinensis 10g


Rou Gui Cortex Cinnamomi 6g


Xi Xin Herba Asari 3g


Sheng Jiang Rhizoma Zingiberis Recens 10g


Gan Cao Radix Glycyrrhizae 6g.

The prescription was taken once a day.


Explanations





▪ Fang Feng, Ge Gen and Ma Huang dispel Wind, Cold and Damp


▪ Dang Gui promotes the circulation of Blood and removes obstructions from the channels; promoting the Blood circulation also can quench Wind


▪ Rou Gui and Xi Xin dispel Wind-Cold and relieve pain


▪ Sheng Jiang and Gan Cao strengthen the Spleen and coordinate the actions of all the other herbs in the prescription.

After 20 days of treatment, the pain had disappeared. A year later she was followed up and she reported that she had experienced no more pain since the treatment.
