Prescription



Main formula



Xiao yao san


Free and Relaxed Powder. This formula is good for harmonising the Liver, smoothing the emotions, tonifying the Liver-Blood and strengthening the Spleen. When the emotion is stable and easy and Liver-Qi circulation is smooth, there will be no pain. However, some other herbs should be added to this formula to promote the Qi circulation and relieve the pain, such as Chuan Lian Zi Fructus Meliae Toosendan 10g and Yan Hu Suo Rhizoma Corydalis 10g.


Alternative formula



Chai hu shu gan san


Bupleurum Soothing the Liver Powder. The advantage of this formula is that it contains some herbs to promote the Qi and blood circulation and to relieve the pain directly; however, it doesn’t have any function for tonifying the Liver-Blood and strengthening the Spleen. Usually Chuan Lian Zi Fructus Meliae Toosendan 10g and Yan Hu Suo Rhizoma Corydalis 10g could be added to the prescription to relieve the pain.


Modifications





1. If there is stagnation of Qi due to the accumulation of Cold, and abdominal pain with a dull sensation, add Wu Yao Radix Linderae 5g to dispel the Cold, regulate the Qi circulation and relieve the pain


2. If there is stagnation of Qi due to its deficiency, add Huang Qi Radix Astragali seu Hedysari 10g to activate the Spleen and tonify the Qi


3. If there is distension of the breast, or formation of cysts in the breast, add Ju He Semen Citri Reticulatae 10g and Zhe Bei Mu Bulbus Fritillariae Thunbergii 10g to promote the circulation of Qi, harmonise the collateral and reduce the cysts


4. If there is a sharp pain, add Yu Jin Radix Curcumae 10g to regulate the Qi and Blood circulation and relieve the pain


5. If the menstruation is irregular, add Yi Mu Cao Herba Leonuri 15g to regulate the menstruation


6. If there are many purplish clots in the menstrual blood, add Mo Yao Resina Myrrhae 10g to eliminate the Blood stasis and relieve the pain


7. If there is restlessness and insomnia due to severe pain, add Xue Jie Resina Draconis 5g and Dan Shen Radix Salviae Miltiorrhizae 10g to promote the circulation of Blood, calm the Shen and relieve the pain


8. If there is nervousness and irritability, add Xia Ku Cao Spica Prunellae 10g and Ju Hua Flos Chrysanthemi 10g to clear the Heat in the Liver and calm the Liver


9. If there is a headache, add Jue Ming Zi Semen Cassiae 10g to relieve the headache


10. If there is nausea and vomiting due to severe pain, add Wu Zhu Yu Fructus Evodiae 3g and Ban Xia Rhizoma Pinelliae Praeparata 10g to regulate the Stomach-Qi and stop the vomiting


11. If the appetite is poor, add Mai Ya Fructus Hordei Germinatus 15g to promote the appetite and improve the digestion.

Yan Hu Suo Zhi Tong Pian Corydalis Stop Pain Tablet


Yi Mu Cao Gao Leonurus Syrup.







Main points


PC-6 Neiguan, LI-4 Hegu, LR-3 Taichong, LR-5 Ligou, ST-28 Shuidao, ST-29 Guilai and SP-6 Sanyinjiao. Reducing method is used on these points. Moxibustion should be applied if there is no formation of Heat in the Liver.


Explanations





▪ PC-6, the Connecting point of the Pericardium channel, is used to unblock the Qi stagnation and calm the Shen


▪ LI-4 and LR-3, ‘the four gate points’, are used to promote the circulation of Qi and relieve pain


▪ LR-5, the Connecting point of the Liver channel, is used to harmonise the collateral, promote the circulation of Qi and relieve the pain


▪ ST-28 and ST-29, the local points, and SP-6, the crossing points of the three Yin channels of the foot, are used together to promote the circulation of Qi and Blood and relieve stagnation so as to relieve the pain

Moxibustion helps promote the Qi circulation and relieve the pain.

Modifications





1. If there is severe abdominal pain, add LR-6 and SP-8, the Accumulation points of the Liver channel and Spleen channel respectively, to harmonise the collateral, promote the circulation of Qi and relieve the pain


2. If the menstruation is irregular, add CV-6 and KI-3 to regulate the menstruation


3. If the menstrual blood is dark with clots, add SP-10 to promote the circulation of Qi and Blood and eliminate Blood stasis


4. If there is hypochondriac pain, add LR-14, the Front Collecting point of the Liver, and GB-40, the Source point of the Gallbladder channel, to harmonise the Liver and relieve the pain


5. If there is a sensation of fullness and oppression in the chest, add CV-17, the Gathering point for the Qi, to promote the circulation of Qi and relax the chest


6. If there is nervousness and headache, add GB-20 to calm the Liver and clear the Heat there


7. If there is restlessness and insomnia due to severe pain, add HT-3 and Extra Anmian to calm the Shen and improve the sleep


8. If there is a poor appetite or nausea and vomiting, add CV-12 and SP-4 to regulate the Stomach-Qi and improve the appetite





Stagnation of Blood



Symptoms and signs


Stabbing pain in the lower abdomen or perineum one or two days before or during the menstrual period, which worsens at night, with a discharge of big purplish blood clots in the menstruation and alleviation of pain after the blood clot discharge are symptoms of stagnation of Blood. There is often cyst formation or even hard mass in the ovary and uterus, a purplish tongue or purplish spots on the tongue and a wiry and choppy pulse.


Principle of treatment


Promote the Qi and Blood circulation, eliminate the Blood stasis and relieve the pain.





Prescription



Main formula



Tao hong si wu tang and shi xiao san


Four-Substance Decoction with Safflower and Peach Pit and Sudden Smile Powder. When these two formulas are prescribed together, their ability to promote the Blood circulation and to eliminate the Blood stasis is very strong. Some herbs to promote the Qi circulation and relieve the pain, such as Xiang Fu Rhizoma Cyperi Rotundi 10g, Yan Hu Suo Fructus Corydalis 10g, Qing Pi Pericarpium Citri Reticulatae Viride 5g and Chuan Lian Zi Fructus Meliae Toosendan 10g, etc., could be added.

Ge xia zhu yu tang


Drive out Blood Stasis Below the Diaphragm Decoction. This formula was originally designed to relieve the pain due to stagnation of Blood in the Middle Burner, but it is also ideal for treating the dysmenorrhoea due to stagnation of Blood in the Lower Burner.


Shao fu zhu yu tang


Drive Out Blood Stasis in the Lower Abdomen Decoction. The specialty of this formula is to warm the channel, dispel the Interior Cold, promote the Blood circulation, eliminate the Blood stasis and relieve the abdominal pain, indicated in dysmenorrhoea due to stagnation of Blood resulting from Cold accumulation or Yang deficiency.


Modifications





1. If there is severe pain, add Xue Jie Resina Draconis 5g to promote the circulation of Blood and relieve the pain


2. If the menstruation is irregular, add Yi Mu Cao Herba Leonuri 15g to regulate the menstruation


3. If there is abdominal pain with a distending sensation and constipation, add Bing Lang Semen Arecae 10g to promote the circulation of Qi and relieve the abdominal distension


4. If cysts or masses form in the ovaries or Uterus, add Ju He Semen Citri Reticulatae 10g and Kun Bu Thallus Laminariae 10g to regulate the circulation of Qi and reduce the swelling


5. If there is profuse bleeding with purplish blood loss during menstruation, or lingering bleeding with purplish blood, add San Qi Radix Notoginseng 10g and Bai Ji Rhizoma Bletillae 10g to promote circulation and eliminate stasis of the Blood and stop the excessive bleeding


6. If there is distending pain in the breast, add Yu Jin Radix Curcumae 10g to regulate the circulation of Qi and Blood and relieve the distension


7. If there is restlessness and insomnia due to pain, add Dan Shen Radix Salviae Miltiorrhizae 10g to calm the Shen and relieve the restlessness


8. If there is nausea and vomiting due to pain, add Wu Zhu Yu Fructus Evodiae 3g and Ban Xia Rhizoma Pinelliae Praeparata 10g to regulate the Stomach-Qi and stop the vomiting.

Tong Jing Wan Regulate Menses Pill


If there is profuse bleeding, add Yun Nan Bai Yao Yunnan White Medicine.

1. If there is hypochondriac or breast pain, add PC-6 and LR-14 to promote the circulation of Qi and relieve the pain


2. If the menstruation is irregular, add CV-4 and KI-3 to regulate the menstruation


3. If there is severe dysmenorrhoea, add LR-6, the Accumulation point of the Liver channel, to promote the circulation of Qi and Blood, remove Blood stasis and relieve the pain


4. If there is nausea and vomiting, add SP-4 and CV-12 to harmonise the Stomach, cause the Stomach-Qi to descend and relieve the vomiting


5. If the appetite is poor, add ST-36, the Sea point of the Stomach channel, and SP-3, the Source point of the Spleen channel, to activate the Spleen and Stomach and improve the appetite.





A 35-year-old woman had been suffering mainly from severe abdominal pain during menstruation for 3 years.

She was the mother of one child and, 3 years previously, she had for the first time had an abortion. She was mildly depressed after this, but was given no treatment. Three months after the abortion, she started to have a slight distending pain in the abdomen during menstruation. At the beginning, the pain was not very severe and was tolerable with the aid of normal painkillers. However, it became worse and worse until she had to take very strong painkillers in order to stop the stabbing pain during menstruation. Her cycle and duration of menstruation were normal.

When she arrived at the practice, she still had a feeling of depression, insomnia, hypochondriac pain, a feeling of oppression over her chest, generalised tiredness, dark and purplish-coloured blood loss during menstruation with big clots, a purplish tongue with a thin and white coating, and a wiry pulse.


Diagnosis


Dysmenorrhoea due to stagnation of Qi and Blood.

Explanations

▪ Huang Qi tonifies the Qi and Dang Gui tonifies the Blood to relieve tiredness. Dang Gui can also prevent damage to the Blood from the strong Blood circulation herbs


▪ Gan Cao relieves spasm and pain. It can also moderate the actions of the other herbs in the formula.


Acupuncture treatment


ST-28 Shuidao, ST-29 Guilai, SP-6 Sanyinjiao, SP-8 Diji, PC-6 Neiguan, LR-3 Taichong, LR-5 Ligou and LR-14 Qimen. Reducing method is used on these points. For the first 3 weeks, treatment was given twice a week; after this, it was given once a week.

After a month of treatment, her dysmenorrhoea had decreased. After 3 months of treatment, her pain had disappeared completely. One year later, she was followed up by telephone and she stated she had not suffered from any further dysmenorrhoea since the treatment.


Accumulation of Cold-Damp



Symptoms and signs


The main symptoms are pain in the lower abdomen with a cold sensation before or during menstruation, which is alleviated by contact with warmth and aggravated by pressure on the lower abdomen, accompanied by scanty and dark menses with blood clots, white-coloured leucorrhoea with no smell, and no vaginal itching. Other symptoms include an aversion to cold, diarrhoea with loose stools, lower back pain with a cold or bearing-down sensation, cold limbs, a white and greasy tongue coating and a deep, slow and slippery pulse.

Warm the channels, eliminate the Cold and Damp, promote the Blood circulation and relieve the pain.

Main formula



Cang fu dao tan tang


Atractylodis and Cyperi Decoction to Guide Out Phlegm. This formula is formed based upon DAO TAN TANG by adding Cang Zhu Rhizoma Atractylodis and Xiang Fu Rhizoma Cyperi.

It can eliminate Damp-Phlegm, activate the Spleen and Stomach, promote the Qi circulation and relieve the pain. However, it cannot warm the Interior nor dispel the Cold, so Gan Jiang Rhizoma Zingiberis 10g, Rou Gui Cortex Cinnamomi 10g and Wu Yao Radix Linderae 5g could be added to stimulate these actions, and Yan Hu Suo Fructus Corydalis 10g, Qing Pi Pericarpium Citri Reticulatae Viride 5g and Chuan Lian Zi Fructus Meliae Toosendan 10g, etc., could be added to promote the Qi circulation and relieve the pain.


Alternative formula



Shen ling bai zhu san


Ginseng–Poria–Atractylodis Powder. This formula can activate the Spleen and Stomach, tonify the Spleen-Qi, eliminate Damp-Phlegm and stop the diarrhoea. Gui Zhi Ramulus Cinnamomi 5g, Chuan Xiong Radix Ligustici Wallichii 10g, Xiang Fu Rhizoma Cyperi Rotundi 10g, Qing Pi Pericarpium Citri Reticulatae Viride 5g, and Xiao Hui Xiang Fructus Foeniculi 5g etc., should be added to eliminate Damp-Phlegm, warm the Interior, harmonise the collaterals and relieve the pain.
