CHAPTER 72 V/Q SCAN IN ACUTE PE: THE PIOPED TRIAL

Value of the Ventilation/Perfusion Scan in Acute Pulmonary Embolism: Results of the Prospective Investigation of Pulmonary Embolism Diagnosis (PIOPED)

PIOPED Investigators. JAMA. 1990;263(20):2753–2759

BACKGROUND

In the 1980s, pulmonary angiography was the gold standard imaging modality for the diagnosis of pulmonary embolism (PE). However, it was resource intensive, requiring skilled angiographer and specialized facilities, and invasive. Lack of access to testing resulted in many missed PEs with significant preventable mortality. Ventilation and perfusion lung scans (V/Q scan) was an emerging noninvasive alternative, showing some diagnostic value, but had not yet been validated in large populations. The PIOPED investigators sought to validate V/Q scan as a reliable diagnostic method for the diagnosis of PE.

OBJECTIVE

To determine the sensitivity and specificity of V/Q lung scan for diagnosis of PE.

METHODS

Prospective, multicenter, observational study conducted at six hospitals between 1985 and 1986.
