CHAPTER 15 TIMI RISK SCORE FOR ACS

The TIMI Risk Score for Unstable Angina/Non-ST Elevation MI: A Method for Prognostication and Therapeutic Decision Making

Antman EM, Cohen M, Bernink PJ, et al. JAMA. 2000;284(7):835–842

BACKGROUND

Patients presenting to the ED with unstable angina (UA) or non-ST–segment elevation myocardial infarction (NSTEMI) represent a heterogenous population with varying risk of adverse outcomes such as death and major adverse cardiac events. Prior to this study risk stratification was based on clinical judgment without objective criteria.

OBJECTIVES

This study aimed to develop a simple, generalizable risk stratification tool for evaluation of patients with UA/NSTEMI as well as to identify patients with different risk profiles and responses to treatments for UA/NSTEMI.

METHODS

Observational analysis of two previously conducted randomized controlled trials.

Patients

7,081 patients with UA/NSTEMI from the efficacy and safety of subcutaneous enoxaparin in non-Q–wave coronary events (ESSENCE) and thrombolysis in myocardial infarction (TIMI) 11B trials. These were double-blinded, placebo-controlled trials that randomly assigned patients to receive enoxaparin or unfractionated heparin.
