Subgluteal Sciatic Block

   Landmarks for the ultrasound-guided subgluteal sciatic nerve block include the greater trochanter (GT) laterally and ischial tuberosity (IT) medially.


    Caudad to the gluteal crease, the sciatic nerve lies along the lateral border of the long head of biceps femoris and anterior to the gluteus maximus muscle.


    Approach and Technique



    1. image   With the patient in Sim’s position, place the ultrasound transducer perpendicular to skin between the ischial tuberosity and the greater trochanter of the femur caudad to the gluteal crease and oriented parallel to the crease.
    2. image   The sciatic nerve lies deep to the gluteus maximus muscle between the IT medially and the femur laterally.
